Public postsecondary education institutions in Louisiana must offer students a voluntary authorization form allowing the institution to disclose protected health information to designated contacts during a mental health crisis or when the student poses a risk to themselves or others

Students may designate a primary contact and optional secondary contact (parent, guardian, or other individual) to receive notifications, including contact name, relationship, phone number, and email address

Information authorized for disclosure includes the general nature of the mental health crisis, institutional actions taken (hospitalization, counseling referral), and follow-up care recommendations—specific diagnoses or treatment details are excluded unless otherwise required by law

Authorization remains in effect until enrollment ends or until the student submits a written revocation request; revocation does not apply retroactively to information already shared

The form is voluntary, does not obligate the institution to notify contacts unless deemed necessary, and disclosed information remains protected under federal and state privacy laws
