Health coverage plans providing maternity benefits must cover voluntary home visiting services, including at least one visit during a newborn's first three months and at least one follow-up visit within three months after the last visit, with plans allowed to cap coverage at five visits from birth to age three.

Coverage may be subject to standard deductibles, coinsurance, and copayments, with a minimum coverage limit of $1,500 per pregnancy or childbirth.

Insurers may choose reimbursement methods including value-based payments, invoice claims, capitated arrangements, or other negotiated payment structures.

Evidence-based home visiting programs must meet U.S. Department of Health and Human Services criteria as listed on the Home Visiting Evidence of Effectiveness registry.

New policies must comply beginning January 1, 2027; existing policies must conform by their renewal date or no later than January 1, 2028.
