Creates the "Medical Freedom and Informed Consent Act" requiring healthcare providers to obtain informed consent before administering any medical intervention, except in medical emergencies

Requires providers to give patients or guardians the Vaccine Information Statement (VIS) before administering vaccinations and document receipt and consent in medical records

Prohibits employers and business entities (public or private) from requiring medical interventions as a condition of employment, enrollment, admission, or service

Prohibits healthcare providers from requiring vaccinations as a condition to be a patient or to receive medical treatment, including cancer treatment, transfusions, or organ transplants

Establishes penalties of at least $5,000 per violation for healthcare providers who violate the law, plus civil remedies including damages, attorney fees, and injunctive relief; requires the attorney general to establish a violation reporting system
