Urges the Louisiana Department of Health to develop and implement an institutional special needs plan (I-SNP) to serve individuals with intellectual and developmental disabilities

The I-SNP would integrate Medicare and Medicaid benefits under a unified care model, operated by a coordinated care organization in partnership with the ARC of Louisiana, managed care entities, and a Medicare Advantage Organization

Services would include comprehensive medical and behavioral health care, long-term services and supports, rehabilitation therapies, vocational and employment assistance, and intensive care coordination

Implementation would follow a phased rollout beginning in high-need parishes with eventual statewide expansion, requiring federal waiver approval from the Centers for Medicare & Medicaid Services

Copies of the resolution to be transmitted to the secretary of the Louisiana Department of Health, the surgeon general, and the governor
