LA SB70

Amends R.S. 40:1227.5 to require a healthcare provider recommendation for disease management services through remote patient monitoring as a baseline qualification for all patients

Reduces eligibility threshold from meeting "any two" criteria to meeting "one" criterion (in addition to the provider recommendation)

Qualifying conditions include chronic conditions defined by CMS (sickle cell disease, mental illness, asthma, diabetes, cancer, heart disease), or two or more hospitalizations/ER visits in the last twelve months due to chronic conditions

Adds two new qualifying categories: pregnant or postpartum women, and infants discharged from a hospital after requiring neonatal intensive care

Effective upon signature by the governor
