Amends R.S. 23:1311 to require workers' compensation petitions to include "sufficient factual statements" putting parties on notice about disputed benefits, rather than simply identifying the specific benefit due

Adds a new petition requirement for a prayer for relief with a concise statement of specific benefits or other relief sought

Changes notice requirements from notifying only the "employer" to notifying all "parties" involved in the claim

Significantly expands when employers or payors can file disputed claims against employees, dependents, or beneficiaries—removing the previous limitation that restricted such filings to fraud allegations under R.S. 23:1208 or appeals of medical director decisions

Employers and payors may now file disputed claims to controvert benefits or concerning any other dispute arising under the workers' compensation chapter
