Health coverage plans in Louisiana must cover FDA-approved injectable drugs for lowering glucose levels or weight loss when prescribed for prediabetes, gestational diabetes, or obesity (BMI ≥ 30 kg/m²)

Coverage requires the drug to be prescribed by a licensed healthcare provider, deemed medically necessary, and included on the insurer's formulary or preferred drug list

Plans may apply standard deductibles, coinsurance, copayments, and prior authorization requirements consistent with other covered benefits

Insurers may require ongoing medical necessity certification and participation in a lifestyle management plan if available

New policies must comply starting January 1, 2026; existing policies must conform by their renewal date or no later than January 1, 2027
