Residents and potential residents of one-star rated nursing homes (per CMS Five-Star Quality Rating System) must receive individual counseling from the Office of Aging and Adult Services explaining the rating and offering alternatives

Alternatives offered must include immediate access to Community Choices Waiver and other home- and community-based services, or immediate transfer to a nursing home rated two stars or higher

One-star nursing home administrators must submit a remediation plan to the Louisiana Department of Health within 30 days of receiving the rating, with quarterly progress reports required

Nursing homes that fail to improve their rating after two years face sanctions up to and including placement on a closure track

Effective August 1, 2026, applying to any nursing home with a one-star rating as of October 1, 2026, until the facility improves to two stars or higher
