Prohibits pharmacy benefit managers (PBMs) from earning income except through transparent management fees that cannot be tied to drug prices, rebates, savings generated, or patient cost-sharing amounts

Bans "spread pricing," where PBMs charge health plans more for drugs than they pay pharmacies, closing a practice that allows PBMs to pocket the difference

Requires PBMs to annually certify compliance to the Commissioner of Insurance, with certification signed by the CEO or CFO

Grants the Commissioner and health plans authority to audit PBMs once per year, with PBMs required to provide detailed reimbursement, claims, and rebate data within 30 days

Authorizes the Commissioner to impose monetary penalties, suspend, or revoke licenses for PBMs that violate these requirements
