Restricts facility fees to services provided on hospital campuses (within 250 yards of main buildings), facilities with licensed emergency departments, or licensed satellite emergency facilities, with violations punishable by fines up to $1,000 per occurrence

Requires health care providers to give patients written notice of facility fees at least 3 days after scheduling appointments made 10+ days in advance, or on-site for shorter-notice appointments

Prohibits out-of-network providers from billing insured patients beyond in-network copayments, coinsurance, and deductibles for "unforeseen out-of-network services" including emergency care, services at in-network facilities by out-of-network providers, and ambulance services

Mandates carriers reimburse out-of-network providers at the carrier's median contracted rate for the geographic region, which constitutes payment in full

Requires carriers to reimburse off-campus hospital outpatient departments and ambulatory surgical centers at rates equivalent to Medicare's non-facility physician office rates for evaluation, management, and ambulatory services
