Providers must submit aggregate reimbursement requests by May 1 each year for the prior calendar year's uncollected amounts, with documentation of collection attempts

Carriers have 120 days to audit submitted requests, notify providers of disputes, and pay 65% of undisputed amounts

Carriers must prohibit preventing providers from collecting patient cost-sharing at time of service and must submit annual public reports on reimbursed and denied amounts
