Requires provider network entities to give participating dentists written disclosure of all third-party health plans accessing the network, applicable fee schedules for each plan, credentialing practices, and administrative policies at initial contracting and upon any changes

Mandates third-party health plans notify customers in clear language when using leased dental networks, provide annual reports on subscriber utilization by network entity, and indicate in provider directories which dentists are accessed through leased arrangements rather than direct contracts

Requires provider network entities to send annual notices to participating dentists by November 15 listing all third-party health plans using their services and patient volume through each plan

Subjects third-party health plans using leased dental networks to public hearings and prior written approval from the Commissioner of Insurance, who must evaluate whether fee schedules and premiums are reasonable and not unfairly discriminatory
