Creates the Permanent Affordability Homeownership Program administered by the Executive Office of Housing and Livable Communities to develop and construct affordable homeownership opportunities statewide

Targets households earning 70-120% of area median income as defined by HUD

Requires housing units to carry affordability restrictions for at least 99 years to qualify as "permanently affordable"

Limits eligible projects to 1-25 housing units, including permanently affordable units within mixed-use developments

Mandates annual reporting to the legislature on funded projects, grant amounts, number of units created, affordability levels, and project sizes
