Requires healthcare providers to supply patients or authorized representatives with complete diagnosis, treatment, and prognosis information within 15 business days of a written request, in language the patient can reasonably understand

Mandates providers furnish copies of health records (including laboratory reports, x-rays, prescriptions, and other technical information) within 15 business days at reasonable cost to the patient

Allows patients to request either their complete health record or only the pertinent portion relating to a specific condition

Permits providers to furnish a summary of records instead of full copies with patient consent

Allows providers to exclude written speculations about patient health conditions from records, except information necessary for informed consent must be provided
