Requires the Massachusetts Department of Public Health to create a surveillance case definition for Long COVID (PASC) aligned with National Academies of Sciences, Engineering, and Medicine standards

Mandates development of a case report form collecting patient data including gender identity, race, year of birth, county/state of residence, primary language, presenting symptoms, infection dates, and treatment information

Active public health surveillance of Long COVID must begin 6 months after enactment, with statewide reports made public at least annually

Department must submit a progress report to the legislature and Joint Committee on Public Health within 1 year of enactment, covering surveillance form development, outreach efforts, and initial results
