Department of Public Health must develop standardized screening forms collecting data on untreated cavities, missing teeth, filled cavities, sealants, and treatment urgency, and provide information about affordable dental care programs

School districts must submit annual reports to DPH by December 31 with data on compliance rates, waivers granted, and pupils with untreated decay
