Establishes a diabetes action plan within the Department of Public Health, subject to appropriation, developed in consultation with the Health Policy Commission, Center for Health Information and Analysis, Group Insurance Commission, and Division of Medical Assistance.

Requires the action plan to set goals and benchmarks to reduce diabetes prevalence, improve diabetes care, and control complications associated with the disease.

Mandates biennial reports to legislative committees by July 1 of each odd-numbered year on diabetes's financial impact, number of affected persons, program participation, and progress toward goals.

Reports must include descriptions of inter-agency coordination, funding needs for diabetes programs, and any legislative recommendations for diabetes prevention, control, and treatment.
