Violations of unauthorized use of physician titles punishable by fines of $100 to $1,000, imprisonment of 30 days to 1 year, or both

Requires all health care practitioners to wear photo ID name tags during patient encounters displaying their name, license type, and license expiration date

Mandates health care advertisements identify the practitioner's specific license type and prohibits deceptive or misleading information about credentials, training, or board certification

Requires supervising MDs or DOs in collaborative practice agreements with non-physician practitioners to post their regular office hours; violations constitute a Chapter 93A consumer protection violation
