Requires the Department of Public Health to create a surveillance case definition for Long COVID (PASC) aligned with the National Academies of Sciences, Engineering, and Medicine definition

Mandates development of a case report form collecting patient demographics (gender identity, race, year of birth, residence, primary language), infection dates, symptom onset dates, and associated diseases including ME/CFS, heart disease, POTS, and diabetes

Active public health surveillance must begin 6 months after enactment, with statewide surveillance reports made public at least annually

Department must submit a progress report to the legislature within 1 year covering surveillance form development, outreach and education efforts, and initial results
