Establishes the Naloxone Purchase Trust Fund to create a universal purchase system for opioid antagonist (naloxone) distribution across Massachusetts healthcare facilities including acute care hospitals, emergency departments, substance use disorder treatment facilities, and community health centers

Requires opioid antagonists be offered to patients with a history of opioid use, diagnosed opioid use disorder, or who have experienced an opioid-related overdose

Funds the trust through transfers from the Executive Office of Health and Human Services via managed care organization assessments, voluntary contributions from third-party payers, and interest earnings

Limits storage and distribution costs to no more than 10% of the fund's total naloxone purchase expenditures when not covered by federal contributions; fund balances carry over between fiscal years

Requires annual reporting on fund collections and expenditures to legislative committees on Ways and Means, Public Health, and Health Care Financing; implementation contingent on receiving necessary federal approvals including waivers under 42 CFR § 433.68
