Authorizes the Commissioner of Insurance to exempt health insurance carriers from certain regulatory requirements under Chapter 176J if the majority of their membership is enrolled in a single rate region, enrolled in subsidized coverage under Chapter 176Q, or if the carrier receives 80% or more of income from government programs

Prohibits contract provisions that grant healthcare providers a guaranteed right of participation in select or tiered network plans, limiting providers' ability to demand automatic inclusion

Prohibits "all-or-nothing" contracting requirements that would force carriers to include all members of a provider group (local practice groups or facilities) in a select network plan together

Prohibits contract terms requiring carriers to place all members of a provider group in the same tier of a tiered network plan, allowing carriers to tier individual providers or facilities separately based on cost or quality
