Mandates early replacement coverage before the 36-month period expires if a physician documents that hearing capability has changed and the current device is no longer sufficient

Covers related services including initial evaluation, fitting, adjustments, supplies (ear molds, batteries, accessories), and repair or replacement of devices that malfunction through no fault of the user

Classifies hearing aids as durable medical equipment with the same cost-sharing terms as other DME, but caps copayments at $200 maximum

Applies to Group Insurance Commission plans, accident and sickness insurance, hospital service plans, medical service agreements, and health maintenance contracts (excludes Medicare supplemental coverage)
