Creates new Chapter 176Z requiring Provider Network Entities that lease or sell access to their dental provider networks to maintain signed written agreements with each participating dentist and disclose which third-party health plans can access the network, applicable fee schedules, and credentialing practices

Mandates third-party health plans reimburse dentists according to contracted fee schedules and provide written notice identifying which Provider Network Entity contract and fee schedule applies to each dental benefit plan

Requires third-party health plans to notify customers in clear language when using leased provider networks and provide annual reports on the number of subscribers receiving services from each network entity

Obligates Provider Network Entities to send annual notices to participating dentists by November 15 listing all third-party health plans using their services and patient volume from each plan

Subjects all third-party health plans using leased dental networks to public hearings and prior written approval from the Commissioner of Insurance, who may deny arrangements with unreasonable fee schedules or excessive, inadequate, or discriminatory premiums
