Establishes a special commission co-chaired by the Secretary of Health and Human Services and a transitional advisory committee representative to review service gaps for transition age youth leaving special education

Commission membership includes 4 legislators, chairs of the joint committee on children, families and persons with disabilities, representatives from The Arc of Massachusetts, transition age youth programs, and affected families

Requires the commission to develop a comprehensive plan addressing agency communication, residential placement timelines (6 months advance identification), geographic equity in services, and improved information sharing between agencies

Plan must be submitted within 1 year of enactment to multiple legislative committees and executive offices, including any budgetary or statutory needs
