Creates a Commonwealth Water Infrastructure Trust Fund managed by the Department of Revenue commissioner to address statewide water infrastructure needs

Imposes additional fees on water/sewer connection permits: $250 for residential properties and $1,000 for commercial/industrial properties, deposited into the trust fund

Distributes fund revenues as follows: 50% returned proportionally to cities/towns for local water infrastructure, 40% to Massachusetts Clean Water Trust, 5% to DEP for economic target area initiatives, and 5% to DOR for administration

Requires municipalities receiving funds to submit annual reports by December 31 detailing water infrastructure projects and expenditures, which DOR must compile and publish online

Directs DEP, in consultation with the Executive Office of Housing and Economic Development, to create an outreach initiative helping municipalities in economic target areas plan and upgrade water infrastructure
