Creates a "permanent affordability homeownership program" administered by the Executive Office of Housing and Livable Communities to fund development of affordable homeownership units statewide

Targets households earning 70-120% of area median income as defined by HUD

Requires housing units to carry affordability restrictions for a minimum of 99 years to qualify as "permanently affordable"

Limits eligible projects to 1-25 housing units, including units within mixed-use developments

Mandates annual reporting to the legislature on program activities, including project descriptions, grant amounts, number of units created, and affordability levels
