Requires the Department of Public Health to establish rules for including opioid maintenance treatment information in the state's prescription monitoring program, where permitted by state and federal privacy laws

Enables prescribers to view a patient's participation in opioid maintenance treatment before issuing new opioid prescriptions (other than the maintenance treatment medication itself)

Mandates opioid treatment programs (as defined under 42 CFR Part 8) to present patients with a consent form at intake and discharge allowing voluntary release of their treatment information to the prescription monitoring program

Consent forms must clearly explain that participation is voluntary but encouraged for better care coordination, and facilities must maintain records of each patient's consent decision
