Requires the Executive Office of Health and Human Services to collect voluntary sexual orientation and gender identity (SOGI) demographic data from residents 16 and older receiving direct services, to reduce health care disparities for LGBTQ+ individuals

Directs the Office of Health Equity to develop training plans and guidelines for state agencies on collecting SOGI information, including cultural competence standards for working with LGBTQ+ populations

Mandates annual reporting of aggregate, de-identified SOGI data to the Office of Health Equity, which must compile findings and submit reports to the Legislature by December 31 each year

Extends SOGI data collection requirements to public school districts and the Department of Higher Education, with data shared annually with the Office of Health Equity

Establishes a phased implementation timeline: health and human services provisions take effect 1 year after enactment; education provisions take effect 18 months after enactment
