Commissioner of Public Health must establish a diabetes action plan within the department, subject to appropriation, developed in consultation with the Health Policy Commission, Center for Health Information and Analysis, Group Insurance Commission, and Division of Medical Assistance.

The action plan must set goals and benchmarks to reduce diabetes prevalence, lower incidence rates, improve diabetes care, and control disease complications statewide.

Biennial reports required by July 1 of each odd-numbered year to the Senate and House Ways and Means Committees, Joint Committee on Health Care Financing, and Joint Committee on Public Health.

Reports must include data on the financial impact of diabetes, number of affected persons, participation in prevention programs, effectiveness of implemented programs, inter-agency coordination, and progress toward established goals.
