Redefines "practice of optometry" to include examination, diagnosis, treatment, and management of diseases, injuries, and disorders of the visual system, eye, and associated structures, plus diagnosis of related systemic conditions

Requires optometrists licensed before certain dates (1984, 1994, or 2021) who have not completed board-approved testing to retake and pass licensing examinations to use diagnostic or therapeutic pharmaceutical agents, including glaucoma and oral medications

Mandates optometrists apply to participate in the state medical assistance program (MassHealth) as a condition of license renewal, for purposes of ordering and referring covered services

Permits optometrists to practice and advertise under trade or service names, provided their individual names are prominently displayed at all practice locations and on prescription forms

Establishes penalties for unlicensed practice: first offense fines of $100-$400 and/or up to 3 months imprisonment; subsequent offenses fines of $400-$1,000 and/or 3-6 months imprisonment
