Amends Chapter 111, Section 51D to require hospital discharge plans include descriptions of any pharmaceutical, rehabilitative, or life-supporting technology, machinery, or equipment needed to maintain medical stability, along with maintenance instructions

Requires hospitals to communicate discharge plans and aftercare provisions to a registered nurse or licensed practical nurse employed at the patient's residence or residential facility before releasing clients with intellectual or developmental disabilities

Mandates that nurses receiving discharge plans for clients with intellectual or developmental disabilities must review life-supporting equipment requirements, consider existing individualized education or service plans, and sign written acknowledgment of full understanding

Requires nurses to advise residential facility personnel, family members, and personal care attendants on discharge plan requirements and hospital recommendations

Directs the Department of Public Health to develop implementing regulations, conduct public awareness campaigns, and post guidance materials on its website; takes effect immediately upon passage
