Requires voters to present valid photo identification before receiving a ballot at polling places in Massachusetts

Directs the Secretary of State to issue free photo identification cards marked "FOR VOTING PURPOSES ONLY" to qualified voters upon request

Accepts the following forms of photo ID: Massachusetts driver's license, state-issued ID card, out-of-state driver's license or ID, U.S. military ID, U.S. passport, or the new state-issued voter ID card

Allows photo IDs expired up to 5 years to remain valid, with voters 65 and older permitted to use expired IDs without time limit

Permits voters without valid photo ID to cast a provisional ballot under existing procedures
