Chronic conditions are defined as health conditions expected to last longer than one year, including intellectual or developmental disabilities

Covered community long-term care services include Adult Day Health, Adult Foster Care, Group Adult Foster Care, Day Habilitation, Home Health Aide, Personal Care Attendant Services, and services under the Frail Elder Waiver

Service authorizations conducted by Aging Services Access Points (ASAPs) for community long-term care services would count as prior authorizations for MassHealth purposes

Service plans may still be modified at the request of the member or primary care provider when a member's condition or needs change
