Establishes the State-Based Health Insurance Subsidies Program to provide subsidies to individuals purchasing health plans through the Maryland Health Benefit Exchange, designed to offset reductions in federal advance premium tax credits under 26 U.S.C. § 36B(b)(3)(A)(iii) for calendar years 2026 and 2027.

Targets individuals who experience increased premium costs due to changes in federal applicable percentage thresholds compared to calendar year 2025 levels, with the goal of maintaining affordability and maximizing enrollment in the individual market.

Authorizes the Maryland Health Benefit Exchange Fund to receive and distribute state-designated funds for the subsidy program during fiscal years 2026 through 2028, with funds from the existing insurance assessment under § 6-102.1 available for both this program and the State Reinsurance Program.

Requires the Exchange to track and publicly report monthly data on subsidy recipients, average subsidy amounts, and the program's impact on individual market insurance rates, with inclusion in annual reports.

Contains an automatic sunset provision: the Act takes effect June 1, 2025, but is abrogated if federal advance premium tax credits are extended for 2026 and 2027; otherwise expires June 30, 2028.
