Authorizes use of electronic health care transaction data from the State-designated health information exchange to support Maryland's participation in the federal AHEAD (States Advancing All-Payer Health Equity Approaches and Development) Model for health care payment reform

Establishes the Population Health Improvement Fund as a special, nonlapsing fund to invest in population health initiatives aligned with statewide health equity targets, including reducing preventable health conditions, addressing health-related social needs, and eliminating health disparities

Authorizes the Health Services Cost Review Commission to assess a uniform amount in hospital rates to fund the Population Health Improvement Fund, with the Maryland Department of Health and the Commission jointly administering the fund

Requires the Department of Health to submit annual reports to the General Assembly beginning July 1, 2026, including program effectiveness measures and health outcome impacts, with a 30-day public comment period on draft reports

Mandates monthly meetings between designated legislators (3 Senators, 3 Delegates) and the Department/Commission on AHEAD Model implementation from July 2025 through December 2026; most provisions sunset December 31, 2030
