Beginning October 1, 2027, the State Board of Physicians may issue a 3-year, non-renewable license to physicians trained and licensed in countries outside the U.S., its territories, Puerto Rico, or Canada

Applicants must hold a medical degree from a WHO-recognized school, complete at least 2 years of postgraduate training equivalent to ACGME-accredited residency, and have practiced medicine for at least 5 of the past 7 years

Applicants must pass USMLE Steps 1, 2, and 3, hold a valid ECFMG certificate, demonstrate English competency, be domiciled in Maryland, and have no pending disciplinary matters

The Board may deny licensure for previous disciplinary actions or competency issues during postgraduate training, and may revoke licenses for practicing outside scope, employment termination, or disciplinary action

The Board must establish regulations providing a pathway to full licensure after successfully meeting license conditions
