Removes the requirement for a licensed health care practitioner's determination or statement when requesting a sex designation change on a Maryland birth certificate; individuals can now make the request through a written statement under penalty of perjury

Adds "unspecified or another" as a third sex designation option (displaying as "X") for new birth certificates, in addition to female and male

Allows new birth certificates to be issued when a court order changes a parent's name listed on the certificate, with the new certificate reflecting the parent's current legal name without showing prior names unless requested

Updates language throughout to use gender-neutral terms such as "birthing parent" and "parent listed on the birth certificate" instead of "mother" and gendered pronouns

Takes effect October 1, 2025
