Carriers covering a biological reference product must immediately add at least one FDA-licensed biosimilar with lower cost-sharing when the biosimilar has a lower wholesale acquisition cost

Prior authorization, step therapy, and pharmacy restrictions that create barriers to accessing these generic drugs or biosimilars are prohibited; applies to policies issued or renewed on or after January 1, 2026
