Individuals enrolled in accredited higher education institutions or residential vocational training programs may return to their former out-of-home placement provider during scheduled breaks, temporary closures, or temporary leaves of absence

Return arrangements require mutual agreement among the individual, the out-of-home placement provider, and the provider agency, plus Department of Human Services approval

Individuals must submit a request to return to a local department of social services at least 30 days before a scheduled break

Local departments must make a recommendation to the Department of Human Services at least 21 days before the break, and the Department must issue a determination at least 14 days before the break
