Establishes the Public Health Abortion Grant Program to provide operating grants to eligible organizations that improve access to abortion care clinical services for uninsured, underinsured, or at-risk individuals in Maryland

Creates a special, nonlapsing Public Health Abortion Grant Program Fund, requiring the Governor to allocate $2,000,000 by budget amendment on or before October 1, 2025, with ongoing annual appropriations beginning in fiscal year 2027

Requires health insurers to transfer 90% of excess funds from segregated abortion coverage accounts to the new grant fund—initially covering plan years 2014-2023 by September 1, 2025, then annually starting July 1, 2026

Mandates that at least 90% of program funds be awarded as grants to eligible organizations, which must be in good standing and have policies consistent with Maryland abortion access laws

Prohibits the Department from collecting identifying information on individuals receiving abortion services or disclosing identifying information about grant recipients' staff and providers; includes automatic abrogation provision if CMS determines the law violates the federal Affordable Care Act by June 30, 2031
