Repeals the Interagency Committee on Aging Services and transfers its responsibilities directly to the Department of Aging, consolidating oversight of senior services under a single agency

Eliminates outdated statutory requirements for the Secretary of Aging to develop congregate housing and assisted living programs for seniors (Sections 10-209 through 10-211, 10-301 through 10-305, and 10-310)

Requires the Department to establish, coordinate, and supervise single points of entry within each planning and service area to provide older adults access to transportation, health, nutrition, housing, financial assistance, and other services at least 5 days per week

Expands eligible services to include assisted living subsidies, home modifications, chore services, climate resilience services, and other supports allowing older adults to age in their chosen location

Beginning June 2025, requires stakeholder engagement for transition planning; individuals enrolled in Senior Care, Senior Assisted Living Subsidy, and Congregate Housing Services Programs will receive person-centered assessments to ensure continued appropriate care (main provisions take effect July 1, 2026)
