Establishes a 3-year Self-Directed Mental Health Services Pilot Program in the Maryland Department of Health for 100 adult Medicaid recipients with behavioral health needs, giving priority to individuals excluded from assertive community treatment, those with multiple disabilities or severe mental illness with trauma-related disorders, and those at risk for institutionalization

Provides participants with individual-directed goods and services including clinical mental health services, medications, transportation, vocational training, technology, homemaker services, short-term rental assistance, respite care, peer support, and care coordination through a person-centered plan

Assigns each participant a support broker to help create person-centered plans, identify services, and obtain payment, along with fiscal intermediary and administrative support

Requires $300,000 annual appropriation for fiscal years 2027-2029, with the Department required to apply for necessary Medicaid waivers and report findings to the Governor and General Assembly by November 1, 2029

Takes effect October 1, 2025 and automatically sunsets on June 30, 2030
