Allows adults with a terminal illness (prognosis of death within 6 months) who have decision-making capacity to request medication from their physician to self-administer to end their life

Requires two oral requests (at least 15 days apart), one written request with two witnesses, confirmation by both an attending and consulting physician, and a mental health assessment if either physician questions the patient's judgment

Protects health care providers from civil, criminal, or professional liability for participating in good faith, while allowing providers and health care facilities to opt out of participation

Classifies death by self-administered medication under this Act as death from natural causes for all legal purposes, including life insurance, health insurance, and annuity contracts

Creates felony penalties (up to 10 years imprisonment and/or $10,000 fine) for forging requests, concealing rescissions, or coercing individuals to request aid in dying
