Department of Human Services must develop a specialized written report form by May 1, 2028, for reporting suspected child abuse or neglect when the victim is currently 18 years or older

The form must document whether the adult survivor prefers the suspected abuse or neglect to be investigated, giving survivors input into the process

The form must include a document for the adult survivor explaining how the information in the report may be used

Department must consult with health practitioners, child abuse survivors, local departments, law enforcement agencies, and the State's Attorney when developing the form

Effective date is October 1, 2026
