Requires Maryland insurers, nonprofit health service plans, and HMOs to cover hormone-related care including hormone replacement therapy, hormone suppression/modulation therapy, related laboratory testing, diagnostic evaluations, and clinical visits for perimenopausal, menopausal, and other hormonal conditions.

Prohibits insurers from denying coverage based on the specific diagnosis (including menopause/perimenopause), patient age, or prior use/nonuse of hormone therapy.

Bars insurers from imposing more restrictive prior authorization, step therapy, or cost-sharing requirements for hormone-related care than for comparable prescription drugs or medical services.

Requires insurers to report annually to the Maryland Insurance Administration beginning January 1, 2028, on claims submitted, claims denied with reasons, and average processing times; the Administration must summarize findings for the General Assembly by July 1 each year.

Takes effect January 1, 2027, applying to all policies issued, delivered, or renewed in Maryland on or after that date.
