Requires insurers, nonprofit health service plans, and health maintenance organizations to reimburse health care providers in a manner that meets annual primary care investment targets established by the State, effective February 1, 2026

Mandates carriers to submit a description of their progress in meeting primary care investment targets when filing premium rates or rate changes with the Maryland Insurance Commissioner

Applies to individual, group, and blanket health insurance policies and contracts delivered or issued for delivery in Maryland

Takes effect October 1, 2026
