Physicians must hold a Certified Medical Director (CMD) certification from the Post-Acute and Long-Term Care Medical Association (PALTMED), or an equivalent credential approved by the Maryland Department of Health, to serve as a nursing home medical director

Physicians already serving as medical directors on or before September 30, 2026 must obtain certification by September 30, 2028, with time-limited extensions available for good cause

A physician may not serve as designated medical director at more than two licensed nursing homes simultaneously

The Maryland Department of Health must establish and maintain a publicly accessible online directory of nursing home medical directors, including each director's name, professional credentials, and certification status

The bill takes effect October 1, 2026
