Both parties must provide information under oath including: full legal names, residences, dates of birth, relationship by blood or marriage, marital status, and details of any prior marriages

Both parties must sign the application and provide Social Security numbers and official government-issued birth certificates or age-verification documents

Maintains existing exception: non-residents of the county where the ceremony will be performed may submit a sworn affidavit instead of appearing in person
