Allows adoptive parents to receive support subsidies for children with special needs, with maximum amounts equal to the foster care rate the child received or would have received, including difficulty of care rates.

Requires the department to complete certification for support subsidies within 30 days of receiving a request and to enter into written adoption assistance agreements covering payment amounts, duration, medical assistance eligibility, and services to be provided.

Permits extension of adoption support and medical subsidy agreements through age 21 if the adoptee has not completed high school or a GED program and is regularly attending school or a disability program on a full-time basis.

Allows continuation of adoption assistance and medical subsidies through Title IV-E funding up to age 19 if the state determines the child has a mental or physical disability warranting continuation.

Ensures adoption assistance and medical subsidies continue even if the adoptive parent moves out of state, during the adoptee's temporary removal from home for delinquency, or to a court-appointed guardian upon the adoptive parent's death.
