Requires physicians who perform abortions to report the procedure to the Michigan Department of Health within 7 days using department-prescribed forms.

Reports must include specific demographic and medical information such as patient age, marital status, race, Hispanic ethnicity, pregnancy history, gestation period, abortion method, and any physical complications.

Prohibits inclusion of patient name, social security number, driver's license number, or any other identifying information in reports; reports must be destroyed after 5 years.

Department must publish annual aggregate statistical summaries of abortion data, including breakdowns by gestation period (4-week intervals from 5-28 weeks), abortions on patients age 17 and under, and reported physical complications.

Violators who disclose confidential identifying information are guilty of a felony punishable by up to 3 years imprisonment or a fine of up to $5,000, or both; effective January 1, 2011.
